An assessment lets you assign any number of points to answer options in multiple-choice questions. The final score is calculated by totaling the points from all the questions. Based on this score, participants are shown a personalized closing screen.
This type of item is ideal for advanced assessments or gamified experiences where outcomes vary based on total points.
You can build an assessment by clicking on the Create a new item button and choosing the Assessment option:

Inside the editor, the left panel lets you edit the structure of your assessment, including the cover page, questions, and results.
The center area is where you can view and edit each card individually. Use the “Preview” button to view the entire assessment as your users will see it.
The right panel includes three tabs: Content, Design, and Settings. Use the Content tab to edit assessment text and media. The Design tab is where you can apply your brand's visual style. The Settings tab gives you control over navigation, display options, and behavior.

Questions
Start by creating some multiple-choice questions. You can assign any number of points to each answer, including negative values. This gives you flexibility in shaping the scoring logic.
Here’s how to assign score values to your answers:
Choose the multiple-choice question from the left-hand side of the editor.
Open the Content tab on the right-hand side and click Set score values:
Assign a value to each answer, and click Save.
For each question, you can provide feedback after submission and shuffle the order of answers.
You can also add all available question types to your assessment. Moreover, you can add a lead form to collect user information, such as name, email, or other custom fields. This is a great option for lead generation!
Closings
Set up different closing screens that appear based on the user’s total score. After you add your closings, you can manually define the score range for each closing, giving you full control over which closing is shown for specific score intervals:
Each user will see the closing that matches their score segment. You can use closings to encourage, inform, or congratulate users, and include an optional call-to-action button.
Settings and customization
In the Design tab, you can change colors, fonts, and more to match your brand. Read more about design customization.
The Settings tab gives you advanced control over your assessment. For example, you can remove the Opinion Stage branding, receive email notifications for new entries, set a closing date, adjust navigation options & more!

Viewing and analyzing results
Once responses start coming in, you can track performance right inside your dashboard. You'll see how many people viewed, started, and completed the assessment, along with engagement metrics like average time and CTA clicks. You can also review individual responses and export results.
