We offer two options to associate the response in a poll or quiz with a user's name (or other contact info).

Option #1: Add a lead form inside your item

  1. Edit your item.

  2. Add a Lead Form to your item.

  3. Configure the lead form fields. You can add any number of fields of different types to associate with the person who answered the poll/quiz (e.g., name, email, phone, etc.).

  4. Click the Save button to save the form.

  5. After configuring the lead form, verify that you Published the item.

Option #2: Tracking based on os_utm_source paramter

This option can only work if you know the names of the users who will be answering your poll/quiz ahead of time.

To track the user who submitted each response, you can add the os_utm_source parameter to the URL of the survey/quiz you send out. This parameter will be added to the response tables in the result dashboard and to the results xls/CSV file you can later export.

For example, if you have a user named Matt Reads, participating in your poll/quiz, you will send him this URL:
