We offer two options to associate the response in a poll or quiz with a user's name (or other contact info).

Option #1 - add a lead form inside a Poll/Quiz

  1. Edit your item (e.g. poll, quiz).

  2. Go to the LEAD & EMAIL CAPTURE section in the form.

  3. Create a New Lead Form or Edit an existing one if you have already created one

  4. Configure the lead form fields - you can add any number of fields of different types to associate the person that answered the poll/quiz (name, email, phone, etc.).

  5. Click the Save button to save the form (you can also preview the lead form here).

  6. After configuring the lead form, verify you save the poll/quiz as well.

Option #2 - tracking based on os_utm_source paramter

This option can only work if you know the names of the users that will be answering your poll/quiz ahead of time.

To track the user that submitted each response, you can add the os_utm_source parameter to the URL of the survey/quiz you send out. This parameter will be added to the response tables in the result dashboard and to the results xls/CSV file you can later export.

For example, if you have a user named matt reads, participating in your poll/quiz, you will send him this URL:
