How Do I Know Who Answered My Item?

We offer two options for associating an item response with a user's name (or other contact information).

Option #1: Add a lead form inside your item

  1. Edit your item.

  2. Add a Lead Form to your item.

  3. Configure the lead form fields. You can add any number of fields of different types to associate with the person who answered the poll/quiz (e.g., name, email, phone, etc.).

  4. Click the Save button to save the form.

  5. After configuring the lead form, verify that you have published the item.

You will later be able to view all the responses associated with the contact info in the results tab of your item.

Option #2: Tracking based on os_utm_source paramter

This option can only work if you know the names of the users who will be answering your item ahead of time.

To track the user who submitted each response, you can add the os_utm_source parameter to the URL of the item you send out. This parameter will be added to the response tables in the result dashboard and to the results xls/CSV file you can later export.

For example, if you have a user named Matt Reads, participating in your quiz, you will send him this URL:
