With Opinion Stage quizzes you can generate up to 5X more leads when compared to a standard form. You can do that by integrating the lead form inside a quiz.
How to add a lead form inside a Poll/Quiz
Edit your item (e.g. poll, quiz)
Go to the LEAD & EMAIL CAPTURE section in the form
Create a New Lead Form or Edit an existing one if you have already created one
Configure the lead form fields - you can add any number of fields of different types to the form (name, email, phone, etc.)
Configure the lead form features - allow users to skip the form, send an email notification on submissions, etc.
Click the Save button to save the form (you can also preview the lead form here)
After configuring the lead form, verify you save the poll/quiz as well

You can view the leads in the Leads report located in the results screen. You can also export the lead information to an xls/csv file by clicking the "Export Results" button at the top of the results screen.
