How to gather contact info such as email, name and phone with a lead form integrated in a quiz or poll

With Opinion Stage quizzes, you can generate up to 5X more leads than a stand-alone form. You can achieve that by integrating a lead form inside a highly engaging quiz.

How to add a lead form inside a Poll/Quiz

  1. Edit your item (e.g. poll, quiz)

  2. Go to the LEAD & EMAIL CAPTURE section in the form

  3. Create a New Lead Form or Edit an existing one if you have already created one

  4. Configure the lead form fields - you can add any number of fields of different types to the form (name, email, phone, etc.) 

  5. Configure the lead form features - allow users to skip the form, send an email notification on submissions, etc.

  6. Click the Save button to save the form (you can also preview the lead form here)

  7. After configuring the lead form, verify you save the poll/quiz as well

Step #1 - select a quiz title that appeals to your audience

Step #2 - ask a few questions in an interactive & visually appealing way

Step #3 - gather leads (e.g. email, phone, name, etc.) before displaying results

Step #4 - View results + add a call to action button

You can either make this mandatory for viewing the results or make it an option (in which case adding a Skip option).

View/Export Leads

You can view the leads in the Leads report located in the results screen. You can also export the lead information to an xls/csv file by clicking the "Export Results" button at the top of the results screen.

You can integrate with your marketing stack to send leads and associated information to any marketing automation or email service. Opinion Stage supports native integrations (e.g. Mailchimp, Hubspot, etc.), Zapier.com integrations, Webhook integrates and an API.
